2011 Calendar Year Forms Required for Troop 557

Medical and Activity Consent Form
Medical forms are required every 12 months. We need part A and part B or the 2011 version of the form completed for all scouts. Part B includes the talent release so that we can share event photos. Part C needs a doctors visit and is not required until our Summer activities. Please take this form to your scouts doctor next time you visit them, and before Summer if your scout will be attending Summer Camp this year.

Troop 557 Dues for 2012
Boy Scouts of America Chief Seattle Council dues and Troop 557 dues are required annually. Each scout needs to pay $40, plus an optional extra $12 (total $52) if a Boys Life Subscription is required.
Please pay dues to the troop treasurer and include a note with your scouts name and the purpose with your dues.
